Incident Management

   Above & Beyond Care is committed to the safety, wellbeing, and rights of all participants. This policy explains how we manage incidents, ensuring they are reported, investigated, and addressed promptly in line with NDIS requirements. 


1. Purpose


To provide a clear process for recording, responding to, and reviewing incidents to prevent harm and improve service delivery. 


2. What is an Incident?


An incident includes any event that causes, or could cause, harm, injury, or risk to a participant, staff member, visitor, or property.


 Examples include:  


  • Accidents or injuries  
  • Medication errors  
  • Abuse or neglect  
  • Property damage  
  • Missing participant or wandering  
  • Any near-miss situations  

3. Responsibilities Staff must: 


  • Immediately ensure safety and provide care if needed.  
  • Report the incident to management as soon as possible.  
  • Complete an Incident Report Form accurately and truthfully.  


Management must: 


  • Review the report and take action to address the incident.  
  • Notify relevant authorities if required under NDIS or legal obligations.  
  • Implement changes to prevent future incidents.  


4. Reporting Process 

  • Record the incident in the official Incident Report Form.  
  • Submit the form to management within 24 hours.  
  • Management investigates and documents outcomes.  
  • Participants, families, or representatives may be informed, depending on the nature of the incident.  


5. Investigation and Follow-Up 


  • Investigations aim to understand what happened, why, and how to prevent recurrence.  
  • Corrective actions are documented and monitored.  
  • Staff may receive additional training if needed.  


6. Confidentiality


All incident information is treated confidentially and stored securely in line with the Privacy Policy. Only authorised personnel access reports. 


7. Continuous Improvement


Incident data is regularly reviewed to identify patterns, reduce risks, and improve service quality. 


8. External Reporting


Serious incidents are reported to the NDIS Quality and Safeguards Commission as required.
